Palliative and End of Life Care Strategy May 2017 - March 2020

East and North Herts Clinical Commissioning Group (CCG) are committed to improving the quality of care for those in need of palliative or end of life care and supporting their carers and families. The need for good quality, community based services is important in reducing the proportion of inappropriate hospital deaths, supporting people to achieve their preferred place of care and ultimately supporting people to achieve their preferred place of death.

This strategy outlines how the CCG proposes to improve palliative and end of life care over the next three years (May 2017-March 2020). It is intended that the strategy is considered as a framework, with detailed action plans subsequently being produced to deliver the overarching principles stated within this strategy.

Local Digital Roadmap (LDR) – Hertfordshire and west Essex

This Local Digital Roadmap (LDR) is the underlying digital strategy for the Hertfordshire and west Essex STP. It will show how we will seek to use technology for the benefit of patients, service users, practitioners and commissioners and support the delivery plan of the STP.

This is the 3rd Year of our Strategic Plan – Planning for Patients and takes account of the NHS England Five year Forward View along with other strategic documents that are referenced in the back of this document. This plan will inform the first year of the local Sustainability and Transformation Plan for 2016 through to 2021.

Primary Care Strategy April 2016

To deliver integrated physical, mental health and social care in the community, which meets patients’ needs in the most appropriate setting.

Primary Care and its component parts are pivotal to sustaining the health economy and must be at the centre of the development of local services.

Together with our partners, our strategy will create a patient-centred, sustainable primary care system which addresses east and north Hertfordshire’s challenges.

We recognise that each of our six localities faces different challenges and will sometimes need to achieve the same goals in different ways. Irrespective of how healthcare is delivered, we will ensure that all patients living in east and north Hertfordshire receive the highest quality care which is easy to access and have the best possible outcomes.

The Hertfordshire Dementia Strategy 2015 - 2019

The Hertfordshire Dementia Strategy 2015 - 2019 has been agreed by Hertfordshire County Council, East and North Hertfordshire Clinical Commissioning Group, Herts Valleys Clinical Commissioning Group and the Hertfordshire Health and Wellbeing Board.

The CCG is determined to improve the quality of care provided to patients, regardless of setting, and will be looking carefully at how best to commission services in 2013/14, and beyond, to achieve sustainable improvements. This plan identifies the challenges the CCG will face in meeting its objectives and details how the CCG will respond.
