From 1 July 2022, Clinical Commissioning Groups will be cease to exist. Commissioning functions and information that has been previously held by East and North Hertfordshire CCG is transferring to the new NHS Hertfordshire and West Essex Integrated Care Board (HWEICB) on 1 July 2022.
HWEICB will become the new data controller. Any questions about the use of data (including patient data) by the new ICB should be directed to hertsvalleysccg.hwecorporategovernance@nhs.net
There are no changes to how local residents access NHS frontline services in Hertfordshire and West Essex as part of these changes.
My plan
Through the development of the patient-held booklet ‘My Plan’, which a number of carers helped to develop, we are encouraging patients to capture what’s important to them, including steps to take in a medical emergency as well as every-day information that is important to their quality of life. We’re trialling My Plan to improve personalised care in several areas across the STP and we’re looking forward to hearing people’s responses at the end of the trial.
One of the carers who has helped to design My Plan, Mike Pye, who cares for his wife Angela, says that he hopes that it will reduce their need to keep re-telling Angela’s story and wishes.
